All participants must read and agree to the following terms before using Play Ball Indoor Turf LLC facilities.
I, the undersigned participant (or parent/legal guardian if participant is under 18 years of age), understand and acknowledge that participating in sports activities, training, and use of Play Ball Indoor Turf LLC facilities involves inherent risks including, but not limited to, physical injury, disability, and death.
I voluntarily choose to participate in activities at Play Ball Indoor Turf LLC and acknowledge that I have been informed of the risks associated with such activities. I understand that it is my responsibility to assess my own physical condition and ability to participate safely.
In consideration for being permitted to use the facilities, I hereby release, waive, discharge, and covenant not to sue Play Ball Indoor Turf LLC, its owners, employees, agents, representatives, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any property belonging to me while participating in such activity or while on the premises.
I consent to receive medical treatment that may be deemed advisable in the event of injury, accident, or illness during my participation. I understand and agree that I am solely responsible for all costs related to such medical treatment, medical transportation, and/or evacuation.
I understand that Play Ball Indoor Turf LLC is not responsible for any lost, stolen, or damaged personal property. I agree to secure all personal belongings and understand that the facility is not liable for any losses.
I agree to comply with all posted rules and regulations of the facility. I understand that failure to follow rules may result in immediate removal from the premises without refund. I agree to conduct myself in a safe and appropriate manner at all times.
I grant permission to Play Ball Indoor Turf LLC to use photographs or video recordings of me (or my minor child) taken during facility use for promotional purposes, including but not limited to social media, website, and marketing materials, without compensation.
I agree to indemnify and hold harmless Play Ball Indoor Turf LLC from any loss, liability, damage, or costs that may occur due to my participation in activities at the facility, whether caused by negligence or otherwise.
I understand that if any portion of this agreement is held invalid, the remainder of the agreement will continue in full legal force and effect.
Important Notice
By signing this waiver (electronically or physically), I acknowledge that I have read this waiver of liability and assumption of risk agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
